Please print out the Market Basket Order Form, complete it with your credit card information (VISA, Mastercard, Discover/NOVUS only) and FAX to: 703-683-3759 or MAIL to: IPHT, 112 South Pitt Street, Alexandria, VA. 22314. You may also make payment by money order or personal check (personal checks must clear before registration is complete). Do not send cash. No COD orders will be accepted.
