PURCHASING POLICIES AND PROCEDURES

TRAINING ORDER: Complete the Training Application Form in the Market Basket.

Payment: At this time, we are not accepting credit card orders over the Internet. Please Internet. Please print out the Market Basket Order Form, complete it with your credit card information (VISA, Mastercard, Discover/NOVUS only) and FAX to: 703-683-3759 or MAIL to: IPHT, 112 South Pitt Street, Alexandria, VA. 22314. You may also make payment by money order or personal check (personal checks must clear before registration is complete). Do not send cash. No COD orders will be accepted.

CANCELLATION: To receive return of Training Deposit, cancellation must be received 20 days prior to the training date. Contact: Mary Hamil Parker, Ph.D. 703-519-8621 or send an e-mail to: IPHT@comcast.net .
